FOCUS FIRE PERMISSION SLIP

Minors (under 18) may participate in 2010 Gaming Generations tournaments with the signed consent of the minor’s parent or legal guardian. Any participant who is considered a minor in his/her jurisdiction must submit this consent form, signed by a parent or legal guardian, upon entry to any Event. Any prizes won by a minor will be awarded to that minor’s parent or guardian identified on the consent form submitted to Gaming Generations

I, as the parent/legal guardian of ____________________________ (“Minor”), hereby consent to Minor’s attendance at one or more tournaments. I understand that the tournaments may involve a video game tournament where an entry fee is charged and where cash and other prizes may be awarded to certain winners. I further understand that the tournaments may feature video game competitions featuring one or more video games that have an Entertainment Software Ratings Board (“ESRB”) rating of “M” or Mature, and that this rating indicates that the ESRB, a recognized industry adopted ratings agency, has deemed content of the video games unsuitable for children under 17 years of age.

I assume all responsibility and liability for any and all damage and injury suffered by or caused by Minor while at the tournaments or any areas surrounding the tournament.

By my signature, I give my permission to allow the above named Minor to attend and participate in any and all tournaments and other activities without restriction or limitation, including but not limited to playing video games with an ESRB rating of “M”, and I hereby release and hold harmless Gaming Generations, affiliates, subsidiaries, suppliers, related companies, advertising and promotion agencies or partners, and any of their respective officers, directors, employees, representatives and agents, from any and all liability that may arise from my decision to allow Minor to participate in the tournaments.

Gaming Generations has Minor’s consent to incorporate any photographs, audio-only or audiovisual recordings in which Minor appears (including, without limitation, news, television and/or motion picture footage) in the tournament and/or Advertisements for use as well as for historical documentation and/or archival purposes, including without limitation, on Producer’s website (e.g., showing Minor’s participation in the tournament); and I, on behalf of Minor, hereby waive any right to any additional compensation therefore, including but not limited to, any excerpt and/or reuse fees and/or residual payments.
